The match ended 3-1 in favour of Pep Guardiola’s side at the Etihad Stadium.
A brace from Phil Foden and a goal from Rodri gave Man City the victory against the Hammers.
Mohamed Kudus did score a goal for West Ham.
The victory means Man City ended the campaign with 91 points from 38 games.
The Cityzens are two points ahead of second position Arsenal, who came from a goal down to beat Everton 2-1 at Emirates Stadium.
